Terms Your completion of this authorization form helps us to protect you, our valued customers, from credit card fraud. All information entered on this form will be kept strictly confidential by Access America Transport. In the event of re-class or re-weighs for LTL shipments Access America Transport reserves the right to charge above card for additional funds. Notification for said extra charges will be made to the card holder in writing prior to charging the card and proof of the additional charges (Weight and Inspection report from the carrier) will be made available at time of notification prior to running the card.
